A Young Person, Adult Carer of a Young Person, or Young Carer are each entitled to their own transition assessment where it is identified that they are likely to have a need for care and support after they, or the young person they care for, reach the age of 18. This includes a child’s needs assessment, a young carer’s assessment, and a child’s carer’s assessment. The term used in this manual for all three is ‘Transition Assessment’. A Transition Plan is developed from this assessment, known in these processes as a 'Transition Plan’. The Transitions Assessment within Mosaic is called an 'Adult My Transition Assessment' and the Transition Plan would be recorded as a ‘My Care and Support Plan’.

If the assessment is done before the young person turns 18, the assessment will be a child’s needs assessment. When the young person turns 18, Lincolnshire County Council (LCC) must decide whether to treat the child’s needs assessment as a needs assessment under the Care Act 2014.

Adult Care’s transitions function for young people with a learning disability is provided by the Intake Team which is part of the Learning Disability Service. The transitions function for young people with a physical disability is provided by the Physical Disabilities Specialist Team.

The Intake Team and the Physical Disabilities Specialist Team work in partnership with Children’s Services to identify and support young people who are moving into adulthood.

Referrals to the Intake Team and the Physical Disabilities Specialist Team can also be made through the Customer Service Centre where there isn’t already an involvement from Children’s Services.
