How to Access Mental Health Social Work

The Mental Health Social Work Team was reconfigured on 9^th January 2023. The service is provided through a s75 partnership between LCC and LPFT, and all requests for adults aged 18 – 65 should be referred to LPFT.

There are three teams co-located with our health care colleagues in LPFT. The teams are West, East and South. All teams use the same referral form, which should be sent to: lpft.socialworkreferrals@nhs.net

Alternatively, a referral can be made via MOSAIC. The referral form can be found as a next action from most workflows within MOSAIC and the next action from the form is to send to the LPFT Triage box.

There is one central duty number for all three teams, offering a virtual reception response for any queries to the service. The number is: 01522 307397.
